Petition for Penalty Relief/Reinstatement/Modification and/or Termination of Probation
Business and Professions Code, section 2661.7 allows a person whose physical therapist or physical therapist assistant license has been revoked or suspended, or who has been placed on probation, to petition the Physical Therapy Board for reinstatement, modification of penalty or termination of probation.
