Retired License

To be eligible for a Retired status license, you must have an active or inactive license in good standing. Licenses that are Delinquent are not eligible for Retired status. Licensees under Retired status are prohibited from engaging in the practice of or assisting in the provision of physical therapy services. Retired status licensees are exempt from the renewal fee and continuing competency requirements. Your license can remain in Retired status for 5 years after which your license will be non-renewable but will retain the status of Retired.

Apply Online

You can request a Retired license through your BreEZe account by selecting 'Request for Retired License Status' under the License Activities section of the Quick Start Menu. Complete the Retired License application questions and pay the $100 fee with a debit or credit card.

Complete your online Application for Retired License at least
4 WEEKS PRIOR to your license expiration date.

Apply By Mail

Complete the Application for Retired License form and return it with your payment of $100 to:

Mail the completed Application for Retired License form with your payment
to the PTBC at least 8 WEEKS PRIOR to your license expiration date.

Return from Retired

You can return your license to Current/Active or Current/Inactive at any time within the 5 years of Retired status By submitting the Application to Restore Retired License form, completing 30 units (one renewal period) of continuing competency education, and submitting one renewal period payment of $300 to:
