Duplicate Wall Certificate

Physical Therapists Only

To request a duplicate or replacement wall certificate submit duplicate wall certificate requests online through your Breeze account. The fee for a duplicate wall certificate is fifty dollars ($50). When you submit your request, please verify your address of record is current and correct.

When requesting a duplicate wall certificate, except due to loss, the original certificate must be returned with your duplicate license request form and payment. Please allow 4-6 weeks to receive your duplicate wall certificate.

Physical Therapist Assistants are not issued wall certificates.

Pocket Licenses

Pocket Licenses will no longer be issued for initial licensure or license renewal. Therefore, duplicate pocket licenses have been discontinued. Licenses must be verified online using the Department of Consumer Affairs License Search, which will provide comprehensive, real-time license information, including enforcement actions
